Before your first appointment
Before your first visit, please make sure that:
- you have completed the relevant pre-appointment forms (women’s medical history form, men’s medical history form or fertility check-up form)
- you have provided the clinic with any relevant records from previous examinations or fertility treatments, such as medical records, information about stored gametes or embryos, laboratory results, semen analysis results and previous treatment protocols. To send your information to us securely, please contact the clinic where you have booked your appointment in advance. If needed, we will provide you with a secure link for sending your documents.
- if a semen analysis is recommended for you, it is completed before your first appointment (book a semen analysis). We recommend 1–3 days of abstinence before providing the sample.
