Understanding the implantation process
After fertilisation, embryos are cultured in our advanced incubators for several days, typically until the blastocyst stage. However, a high-quality embryo is only part of the equation. For pregnancy to begin, the uterine lining must be completely receptive.
Endometrial receptivity depends on a carefully balanced hormonal environment. The developmental stage of the embryo and the uterine lining must be perfectly synchronised. We ensure that the thickness of the uterine lining is optimal, giving the embryo the best possible conditions for implantation.
The role of progesterone and hormonal preparation in embryo implantation
Supporting the development of a receptive endometrium
Adequate and well-timed progesterone exposure is essential for the uterine lining (endometrium) to become receptive and allow successful embryo implantation. After IVF stimulation, progesterone supplementation is always required to support implantation and start a pregnancy. Progesterone can be administered vaginally, orally, or as a subcutaneous injection.
Frozen embryo transfer (FET) can be performed either in a natural ovulatory cycle or in a hormonally prepared cycle. In both approaches, optimal timing is crucial. The developmental stage of the embryo must be carefully synchronized with the readiness of the endometrium. In practice, this means that cleavage-stage embryos are transferred earlier in the cycle than blastocysts. The goal is to perform the transfer during the period when the uterus is most receptive—known as the “window of implantation.” In most FET cycles, embryos are transferred at the blastocyst stage.
An ovulatory cycle may occur naturally or be supported with medication. The timing of embryo transfer is calculated based on the moment of ovulation, which is identified using ovulation tests and/or ultrasound monitoring. In some cases, ovulation is intentionally triggered with an hCG injection. Progesterone supplementation is started shortly after ovulation, and the embryo transfer is scheduled according to the embryo’s developmental stage.
Additional treatments to support implantation
While the treatments described above are sufficient for most patients, some situations require additional support. If you have experienced repeated implantation failure or if the endometrium does not develop adequately, we may consider advanced supportive treatments to improve implantation conditions:
- GnRH agonist injection: Administered the day after embryo transfer to support hormonal balance.
- Support for blood flow and endometrial growth: If endometrial development is insufficient, low-dose acetylsalicylic acid (aspirin), mini-heparin, or PRP (platelet-rich plasma) treatment may be considered.
- Immune modulation: In cases of recurrent implantation failure, prednisolone or intralipid infusions may be used to help balance the immune response and support implantation.
Before recommending these, your doctor will review your history and conduct necessary tests (such as clotting screens or uterine biopsies) to ensure safety and efficacy.
Laboratory methods to support embryo implantation
In our laboratory, we use well-established methods that support the delicate interaction between the embryo and the uterus.
EmbryoGlue: This is a transfer medium rich in hyaluronic acid, a substance that occurs naturally in the body. It can act as a supportive “bridge” between the embryo and the uterine lining and may help the embryo attach more effectively during the initial stages of implantation.
GM-CSF (Growth Factor): This is a natural protein that can be added to the culture medium to support early embryo development. This approach may be particularly beneficial in cases where embryo development has previously been slow or where there is a history of recurrent miscarriages.
