TESA (Testicular Sperm Aspiration) is a modern and gentle procedure used to retrieve sperm directly from the testicle when no sperm are present in the semen. It can be an important step toward genetic parenthood for men whose sperm cannot reach the ejaculate—often due to a blockage or previous surgery.

TESA is primarily used in cases of:
Obstructive azoospermia
Sperm are produced normally in the testicles, but a physical blockage prevents them from reaching the semen.
A common example is men who have previously undergone sterilization (vasectomy).
In rare cases, TESA may also be used when:
fertilization has previously failed with ICSI
sperm show very high DNA fragmentation
In these situations, sperm retrieved directly from the testicle may offer a better chance of fertilization.
Before the procedure, a medical evaluation is carried out—often including hormone testing—to assess testicular function.
When sperm are found, they are frozen and later used for ICSI (intracytoplasmic sperm injection), an IVF technique in which a single sperm is injected directly into an egg.
Recovery is usually quick. Most patients can return to normal daily activities within one to two days.
Pain is generally mild and can be managed with standard pain relief
TESA does not affect testosterone levels or hormonal balance
Testicular function is not affected in the long term
Because the number of retrieved sperm is limited, ICSI is always used for fertilization. Despite this, TESA can be the most effective—and sometimes the only—option for couples who otherwise have a very low chance of achieving pregnancy.
