Fallopian tube patency test (HSSG)

A fallopian tube patency test—HSSG (hysterosalpingo-sonography)—shows whether the fallopian tubes are open and functioning properly. Open tubes are essential for pregnancy, as sperm need to reach the egg, and the fertilised egg must then be able to travel onward to the uterus.

In a healthy fallopian tube, tiny cilia (“hair-like structures”) help guide sperm toward the egg at ovulation. After fertilisation, the movement changes and supports the fertilised egg on its journey to the uterus—a process that takes about five days.

A patency test is especially recommended if you have previously had a chlamydia infection, endometriosis, or abdominal surgery, as these conditions can affect fallopian tube function.

When is the examination performed?

The examination is scheduled early in the menstrual cycle, usually on days 6–10—after menstruation but before ovulation. We help you book the correct time. Before the examination, you need to ensure:

That you are not pregnant
That you do not have an ongoing infection

To reduce any menstrual cramp–like discomfort, we recommend taking pain relief about one hour beforehand, for example:

Ibuprofen 400–800 mg, or
Paracetamol 1 g (if you cannot tolerate anti-inflammatory medication)

How an HSSG is performed

Ultrasound

The examination begins with a vaginal ultrasound.

Catheter placement

A thin catheter is then gently inserted into the uterus. At the tip of the catheter is a small balloon, which is filled with fluid to keep it securely in place.

Introduction of fluid and air

A mixture of sterile saline solution and air bubbles is introduced through the catheter. Using ultrasound, we follow the movement of the bubbles.

If the bubbles are seen near the ovary, the fallopian tube is open.
Sometimes a temporary muscle spasm can cause a false blockage, which usually resolves during the examination.

The examination also provides information about:

The shape of the uterus
The appearance of the uterine lining

In some cases, a special contrast medium (Foam Kit) is used. The physician decides which method is most suitable for you.

The entire examination takes about 30 minutes.
You receive an initial assessment immediately. If the fallopian tubes are open, you can try to conceive during the same cycle.

Light, blood-tinged discharge on the same day is common.
Contact us if you experience heavy bleeding, fever, or increasing pain in the days following the examination.

After the examination – what happens next?

If both fallopian tubes are open, pregnancy can often be achieved naturally.
If one fallopian tube is open, there is still a chance of becoming pregnant spontaneously.
If both tubes are blocked, IVF is usually recommended, as fertilisation cannot take place in the fallopian tubes.
